It is important to understand that some pets respond to grooming differently, even with the best efforts. We make every effort to make it a safe and positive experience. Sometimes pre-existing conditions arise unforeseen to our groomers, such as skin allergies, shampoo allergies, moles, clipper sensitivity, matting and tangles, fleas, fear, behavioral issues, bone or joint sensitivity, heart condition, seizures, etc. It is helpful that you, as the pet owner, inform us of anything that you are aware of prior to grooming. We cannot groom your dog if: Your pets have raw wounds Your pets are in heat or in the middle of menstrual cycle Your pets are pregnant Fleas & Ticks: In order to protect transfer of fleas from one appointment to the next, it is our policy to reschedule appointments immediately following a pet with fleas, to flea bomb the van and tools, and for the groomer to shower and change uniform. If you suspect your pet has fleas or ticks, please inform us prior to the service. Please note that parasites are a health hazard to your pet as well as to humans. If fleas or ticks are found during the grooming process, your pet will be given a flea bath and you will be charged an additional fee of $50.Initial* Reset signature Signature locked. Reset to sign again Matted Coats and Excessive Undercoats: Unmaintained coats cause a variety of health problems for your pet. Fleas, ticks and other parasites may be lurking in the coat causing further skin infections. The skin underneath can be raw and inflamed. Severely matted coats will require a shave down. We are happy to show you how to care for your pet’s coat in the future, however we cannot brush out a coat that is already severely matted due to the stress and the pain it will cause your pet. Your pet may be nicked or cut because we have to work so closely to the skin to remove the coat. We are very careful, but it is possible an injury may occur. Shaving a severely matted coat is a very delicate process and an additional fee will be charged. The rate is $40 per each additional half hour (30 min) above what a typical groom for similar size pet and services would be.Initial* Reset signature Signature locked. Reset to sign again Current Vaccinations: Puppies and Kittens must have completed all their shots before the first groom. By signing this contract, owners attest their pets are current on Rabies, Distemper and Parvo-Virus. Proof of vaccination shall be provided upon request. We are required by Santa Clara County Health Department to verify Rabies vaccination is current. If your dog is exempt from Rabies shot from the county, please provide documentation for the exemption. Re-washing Policy: Please walk your pet for a minimum of 5 minutes prior to the grooming appointment. Pets get nervous about grooming and it will cause them to have a need to eliminate waste and urinate. Your pet will feel much more comfortable if allowed to relieve themselves prior to the appointment. Keep in mind your pet will not be taken outside for the duration of the groom and that most appointments take 1 to 2 hrs. Should they have accidents and requires the groomer to re-do the wash, we will need to charge the extra bath fee. The price range of the bath is from $30 to $60 depending on the size of your dog and the amount of time spent. Please be mindful that this fee is to account for the extra time and work as well as the interruption it causes the groomers. Aggressive or Dangerous Pets: Owners MUST inform us if your pet(s) bites, has bitten, or is aggressive to people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet, and protects both the pet and the groomer. We reserve the right to refuse/stop services for such pet(s) at any time before or during the grooming process. If this happens you will be charged a $50 trip fee. Health or Medical Problems & Senior Pets: Grooming procedures can sometimes be stressful and can expose hidden medical problems or aggravate a current one during or after the groom. Because senior pets and pets with health problems have a greater chance of injury, the groomer’s first objective for these pets is cleanliness and comfort. Please let the groomer know of any health concerns your pet may have. We reserve the right to refuse/stop services for such pet(s) at any time before or during the grooming process. If this happens you will be charged a $50 trip fee. Panting, Drooling, & Similar Signs of Stress: Should your dog or cat exhibit any signs of being over stressed, we will stop the groom. You will be called out to the van to discuss what we are observing, and with you, determine if we should continue. Your pet’s well being is our first priority. Accidents: There is always the possibility an accident could occur. Grooming equipment is sharp, even though we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. Every effort will be made to insure your pet is groomed as safely as possible. In the case of an injury does occur to your pet during the groom, the groomer will take care of the injury according to our safety and accident policy. And in the case of a serious injury he/she will inform you immediately. Appointment Cancellation, Tips, Payment & Returned Checks: If you need to reschedule your appointment please contact us within 24 to 48 hours of the appointment, otherwise you may be charged for the appointment. Tips are always appreciated but NEVER expected. If a groomer exceeds your expectations a tip of 10-20% is typical. 100% of tips goes to your groomer. Payment for groom services is due at the time of service. We accept cash, check, credit, and debit cards. We do NOT provide change if pay in cash. For your convenience and with prior arrangement, you may put your credit card on file with us. We will charge your card for services the following day. Reset to sign again Hold Harmless Agreement: I hereby release Aussie Pet Mobile South Bay from any responsibility of accidents, parasites, medical problems or other problems that may be uncovered while the grooming service is being performed. I hold harmless Aussie Pet Mobile South Bay from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will overwrite any and all prior signed contracts or releases. It is further understood this clause applies to any and all pets groomed.
